- Abstract
- Spontaneous intracranial hypotension is typically manifested with postural headache. The major cranial magnetic resonance image(MRI) abnormalities are diffuse pachymeningeal gadolinium enhancement, subdural fluid collections, and evidence of descent of the brain. Besides these findings, there have been a few reports about spinal MRI changes such as abnormal widening of perineural root sleeves at leakage points, thickened dural enhancement, and spinal subdural effusion. A 41-year-old man presented with severe orthostatic headaches, preceded by minor head trauma of one-month interval. Brain MRI and cerebrospinal study revealed typical findings compatible with spontaneous intracranial hypotension. Additionally, lumbar spine MRI demonstrated abnormal multiangled dural sac deformity suggesting collapse of dural sac and prominent epidural venous signals, which might result from hydrostatic change in intraspinal space. We report a case of spontaneous intracranial hypotension with spinal dural sac deformity.
