당뇨, 간경화가 있는 54세 남자가 고열과 혼미로 내원하였다. 뇌전산화단층촬영, 자기공명영상에서 뇌실로 파열된 뇌농양과 뇌실염, 수막뇌염 소견이 있었다(Fig.). 광범위항생제를 투여함에도 뇌실내에 농양이 증가하고 수두증이 진행하여 뇌실외배액술을 시행하였다. 수술 다음 날, 신경학적 진찰에서 의식은 계속 혼미하였으며, 눈꺼풀을 올려 진찰 시 안구가 양측을 향해 번갈아 느리게 수평으로 배회하는(3.3회/10초) 순회눈운동이 있었다. 순회눈운동은 이틀간 지속되다가 사라졌으며, 수술 6일 후 환자의 의식상태는 기면상태로 호전되었다. 순회눈운동은 종종 혼수 환자에서 관찰되며, 이는 양측 대뇌의 자극이 줄어들어 상대적으로 정중곁교뇌망상체와 위쪽연수의 안구운동기전이 우세하게 되어 나타나는 것으로 알려져 있다[1]. 본 환자에서 국소적인 뇌간의 병변이 보이긴 하지만 순회눈운동을 동반한 것으로 보아 혼수의 원인이 양측 대뇌반구의 문제에서 기인하였음을 알 수 있으며, 이러한 소견은 혼수상태의 환자에서 병소를 감별하는 데 도움이 될 수 있다[2].
